ACCEPTANCE OF E-MAILED OR FAXED DECLARATION OF MEDICAL PROFESSIONAL FOR EXTENSIONS OF EMERGENCY GUARDIANSHIP DECREES PURSUANT TO 20 Pa.C.S.A. § 5513
ORDER OF COURT
AND NOW, this 2nd day of November, 2016, it is hereby ORDERED and DECREED that, when considering a request for an Order extending the appointment of an emergency guardian of the person beyond seventy-two (72) hours pursuant to 20 Pa.C.S.A. § 5513, the Court will accept an e-mailed or faxed declaration of a medical professional supporting such request if the following conditions are met:
1. The declaration must be signed by the same medical professional who provided testimony at the hearing which resulted in the appointment of an emergency guardian.
2. The declaration must be signed by at least one witness.
3. The party requesting the extension shall be responsible for filing the original declaration bearing the medical professional’s signature within five (5) business days of the entry of an extension Order.
