RULE N1920.45. COUNSELING. NOTICES. COSTS. FORM.
(a) A request for counseling may be made by either party. A suggested form of request and order of court is attached to these rules. The party obtaining an order for counseling shall promptly notify his spouse of the order by sending a copy of the order and a copy of the List of Qualified Professionals by regular mail and file a certification of notice with the Clerk. The cost of counseling shall be borne by both parties in equal amounts unless otherwise directed by the Court.

ORDER OF COURT
AND NOW, this day of , 19 , plaintiff and defendant are hereby ordered to participate in a maximum of three (3) counseling sessions with a qualified professional of their choice. The counseling sessions are to be completed within 90/120 days from the date of the filing of the complaint, unless extended by the Court for cause shown. The counselor shall file a report with the Court stating whether the parties did or did not attend.
The party obtaining this Order is directed to send a copy of this Order and the "List of Qualified Professionals" to his/her spouse by regular mail and file a certification of such service with the Clerk.
